Tesamorelin occupies a narrow position among agents that act on the growth hormone axis. Unlike growth hormone itself, which is given as replacement, it stimulates the pituitary to release the hormone in pulses, so the downstream increase in insulin-like growth factor 1 depends on intact somatotroph function. Other peptides in the same family include shorter GHRH fragments and synthetic secretagogues with different stability profiles. Several points remain unresolved, including whether the reduction in visceral fat translates into fewer cardiovascular events, what happens to metabolic markers after long-term use, and how the drug compares with lifestyle or surgical approaches.
Tesamorelin is a synthetic peptide that belongs to the growth hormone-releasing hormone family and contains the same forty-four amino acid sequence as endogenous GHRH, extended at the amino terminus by a trans-3-hexenoyl group. That small fatty acid modification protects the peptide from rapid cleavage by dipeptidyl peptidase-4, the enzyme that shortens the half-life of native GHRH to only a few minutes. Chemically the compound is produced by solid-phase peptide synthesis, purified by chromatography, and supplied as a sterile lyophilized powder for reconstitution.

Receptor-level activity begins when the peptide binds the GHRH receptor, a class B G-protein-coupled receptor found on pituitary somatotroph cells. Occupancy triggers Gs-mediated activation of adenylyl cyclase and a rise in intracellular cyclic AMP, which in turn promotes synthesis and pulsatile release of growth hormone. Because the compound acts upstream of the growth hormone axis rather than supplying hormone directly, its effect depends on intact pituitary function. Binding studies in cell culture and animal models have established this pathway; the detailed kinetics of receptor recycling in humans remain less well characterized.
Physicochemical behavior is dominated by the peptide backbone. The molecule is hydrophilic and carries a net positive charge near neutral pH, owing to several arginine and lysine residues. In solution it adopts a largely unstructured conformation, and aggregation is a known concern for peptide products of this size. Oxidation of methionine and deamidation of asparagine or glutamine residues are the principal chemical degradation routes. These liabilities shape how the material is formulated, handled, and analyzed, and they explain why lyophilized presentations are common in research settings.
Tesamorelin is a synthetic peptide analog of growth hormone-releasing hormone, built from 44 amino acids. Its sequence follows the natural human GHRH(1-44) backbone, with a trans-3-hexenoyl group attached to the N-terminal tyrosine. This modification blocks recognition by dipeptidyl peptidase IV, the enzyme that rapidly truncates the native hormone in circulation. The result is a molecule with a substantially longer plasma residence time than unmodified GHRH, which makes it practical for clinical and laboratory study.
Bethlem myopathy is predominantly an autosomal dominant myopathy, classified as a congenital form of limb-girdle muscular dystrophy. There are two types of Bethlem myopathy, based on which type of collagen is affected. Bethlem myopathy 1 (BTHLM1) is caused by a mutation in one of the three genes coding for type VI collagen. These include COL6A1, COL6A2, and COL6A3. It is typically autosomal dominant, though uncommonly can be autosomal recessive. Bethlem myopathy 2 (BTHLM2), formerly known as myopathic-type Ehlers–Danlos syndrome, is caused by a mutation on the COL12A1 gene coding for type XII collagen. It is autosomal dominant. In 2017, an international workshop proposed a redefined criteria and naming system for limb-girdle muscular dystrophies. Bethlem myopathy 1 (collagen VI) was included into the proposed list and renamed LGMDD5 for autosomal dominant mutations and LGMDR22 for recessive mutations. Bethlem myopathy 2 (collagen XII) was not addressed. Gowers's sign, toe walking, multiple contractures of the joints (especially the fingers: 'Bethlem sign'), skin abnormalities, and muscle weakness (proximal more than distal) are typical signs and symptoms of the disease. Initially, in early childhood, there may also be joint laxity. There is no cardiac involvement in either Bethlem myopathy 1 or 2, which helps to differentiate it from Emery–Dreifuss muscular dystrophy. Currently there is no cure for the disease and symptomatic treatment is used to relieve symptoms and improve quality of life.
=== Suicide === The US Food and Drug Administration (FDA) requires all antidepressants, including sertraline, to carry a boxed warning stating that antidepressants increase the risk of suicide in persons younger than 25 years. This warning is based on statistical analyses conducted by two independent groups of FDA experts that found a 100% increase of suicidal thoughts and behavior in children and adolescents, and a 50% increase in the 18–24 age group. Suicidal ideation and behavior in clinical trials are rare. For the above analysis, the FDA combined the results of 295 trials of 11 antidepressants for psychiatric indications to obtain statistically significant results. Considered separately, sertraline use in adults decreased the odds of suicidal behavior with a marginal statistical significance of 37% or 50% depending on the statistical technique used. The authors of the FDA analysis note that "given the large number of comparisons made in this review, chance is a very plausible explanation for this difference". The more complete data submitted later by the sertraline manufacturer Pfizer indicated increased suicidal behavior. Similarly, the analysis conducted by the UK MHRA found a 50% increase of odds of suicide-related events, not reaching statistical significance, in the patients on sertraline as compared to the ones on placebo.

=== Reactions with electrophiles === The sulfur center in DMSO is nucleophilic toward soft electrophiles and the oxygen is nucleophilic toward hard electrophiles. With methyl iodide it forms trimethylsulfoxonium iodide, [(CH3)3SO]+I−:
=== University of Queensland === Commencing in March 1916 at the University of Queensland he studied science, specialising in biology under Thomas Harvey Johnston, where he received training in animal morphology. He was awarded his Bachelor of Science in 1919. During his honours course, he produced his first research paper in 1918: an anatomical study of the echiuroid worm (pseudobonelia). He received his Master of Science in 1921, at the age of 25. He wanted to study medicine, but there was no medical school in Queensland, so instead continued into zoology. In 1920 he was the beneficiary of a Walter and Eliza Hall Fellowship in economic biology. He worked with scientists researching the control of the blowfly and prickly pear in Queensland, and was involved in the campaign to eradicate hookworm. Tiegs was the first graduate of the University of Queensland to be awarded the degree of doctor of science.

Degenerative suspensory ligament desmitis, commonly called DSLD, also known as equine systemic proteoglycan accumulation (ESPA), is a systemic disease of the connective tissue of the horse and other equines. It is a disorder akin to Ehlers–Danlos syndrome being researched in multiple horse breeds. Originally thought to be a condition of overwork and old age, the disease is now recognized as hereditary and has been seen in horses of all ages, including foals. The latest research (2010) has led to the proposed renaming of the disease from DSLD to ESPA because of the systemic and hereditary components now being found.
== Notable users == King Chulalongkorn of Thailand (1853–1910) used the drug for a period after 1893 to relieve what may have been a mix of depression and unspecified illnesses. He was reported by his doctor to have been taking one bottle per day during July 1894 although this was reduced after this time. Montgomery Clift (1920–1966), American actor. André Gide (1869–1951) was given chloral hydrate as a boy for sleep problems by a physician named Lizart. Gide states in his autobiography If It Die... that "all my later weaknesses of will or memory I attribute to him." William James (1842–1910), psychologist and philosopher, used the drug for insomnia and sedation due to chronic neurosis. The Jonestown mass murder-suicides in 1978 involved the communal drinking of Flavor Aid poisoned with diazepam, chloral hydrate, cyanide, and promethazine. Mary Todd Lincoln (1818–1882), wife of American president Abraham Lincoln, became addicted in the years after her husband's death and was committed to an asylum. Marilyn Monroe (1926–1962) died from an overdose of chloral hydrate and pentobarbital (Nembutal). Friedrich Nietzsche (1844–1900) regularly used chloral hydrate in the years leading up to his nervous breakdown, according to Lou Salomé and other associates. Whether the drug contributed to his insanity is a point of controversy. Dante Gabriel Rossetti (1828–1882) became addicted to chloral, with whisky chasers, after the death of his wife Elizabeth Siddal from a laudanum overdose in 1862. He had a mental breakdown in 1872.
Laureates have been awarded the Nobel Prize in a wide range of fields that relate to physiology or medicine. Only one woman, Barbara McClintock, has received an unshared prize in this category, for the discovery of genetic transposition. Mario Capecchi, Martin Evans, and Oliver Smithies were awarded the prize in 2007 for the discovery of a gene targeting procedure (a type of genetic recombination) for introducing homologous recombination in mice, employing embryonic stem cells through the development of the knockout mouse. In 2009, the Nobel Prize was awarded to Elizabeth Blackburn, Carol W. Greider and Jack W. Szostak of the United States for discovering the process by which chromosomes are protected by telomeres (regions of repetitive DNA at the ends of chromosomes) and the enzyme telomerase.
The inverted terminal repeat (ITR) sequences comprise 145 bases each. They were named so because of their symmetry, which was shown to be required for efficient multiplication of the AAV genome. The feature of these sequences that gives them this property is their ability to form a hairpin, which contributes to so-called self-priming that allows primase-independent synthesis of the second DNA strand. The ITRs were also shown to be required for both integration of the AAV DNA into the host cell genome (19th chromosome in humans) and rescue from it, as well as for efficient encapsidation of the AAV DNA combined with generation of a fully assembled, deoxyribonuclease-resistant AAV particles. With regard to gene therapy, ITRs seem to be the only sequences required in cis next to the therapeutic gene: structural (cap) and packaging (rep) proteins can be delivered in trans. With this assumption many methods were established for efficient production of recombinant AAV (rAAV) vectors containing a reporter or therapeutic gene. However, it was also published that the ITRs are not the only elements required in cis for the effective replication and encapsidation. A few research groups have identified a sequence designated cis-acting Rep-dependent element (CARE) inside the coding sequence of the rep gene. CARE was shown to augment the replication and encapsidation when present in cis.

It is a laboratory-made peptide of forty-four amino acids whose sequence matches human growth hormone-releasing hormone, with a modified amino terminus. The modification is a short unsaturated fatty acid chain attached to the first residue. This change slows enzymatic breakdown and lengthens the time the peptide stays active in circulation.
